- Collect a physical copy from the Chief Medical & Health Officer (CMHO) office in your district.

Include self-attested copies of:
10th & 12th Mark Sheets and Certificates
Recent Passport-Size Photographs
Identity Proof (e.g., Aadhaar Card)

Any supporting document for age relaxation
₹20 Postal Order (Only for General, OBC, EWS categories)
